Details of National Programmes

1. National Programmes implemented through division are as under

• National Programme Name: National Rabies Control Programme (NRCP)

*Programme details:

NRCP was approved and rolled out in the entire country under the 12th Five Year Plan. NCDC is the nodal agency for implementing the health component. State level activities of the program are being implemented through the National Health mission since FY2019-20. The programme has now been approved under SFC for FY 2021-26 under Scheme E of umbrella scheme of NCDC.

Objective: To prevent and control deaths due to Rabies in Humans and progressively achieving the Global target for “Rabies- Zero by 2030.

Strategies of the Programme:

1. Capacity Building of States and district level manpower for prevention and control of rabies
2. Promote of cost effective Intra-dermal rabies vaccines for Rabies Post Exposure Prophylaxis.
3. Strengthen rabies diagnostics.
4. Strengthening Surveillance of animal bites and rabies cases  
5. Information, Education & Communication 
6. Intersectional coordination 
7. Operational Research

Implementation of the Programme:

1. All State and District level activities of the program are being implemented through the National Health mission since FY 2019-20.
2. State and District level activities are-

• Provision of procurement of Anti-Rabies Vaccine/Anti-Rabies Serum for animal bite victims through National Free Drug Initiative
• Capacity building through trainings of Medical Officers and Health Workers under NRCP
• IEC/BCC under NRCP: Rabies Awareness and Do’s and Don’ts in the event of Animal Bites
• Monitoring and Surveillance (review meetings, Travel) and printing of formats for Monitoring and Surveillance

• National Programme Name: Programme for Prevention and Control of Leptospirosis (PPCL)

Background:

To address the rising burden of Leptospirosis, the Govt. of India launched a Pilot Project on Prevention and Control of Leptospirosis as a “New Initiative” under XI Five-year Plan. The Strategy adopted in the Pilot Project was found to be feasible and reproducible. With the lessons learnt in the Pilot Project the Govt. of India has launched the Programme for Prevention and Control of Leptospirosis during 12th Five-year plan in the endemic States/UTs (5 States& 1 UT) viz. Gujarat, Kerala, Tamil Nadu, Maharashtra, Karnataka and UT of Andaman & Nicobar Island. SFC approval for the program was obtained on 15.07.2013 with total budget outlay of Rs.3.753 Crores for the plan period.

Based on epidemiological analysis of IDSP data it is evident that cases are being regularly reported from States such as Uttar Pradesh, Assam, Goa and UTs of Dadra Nagar Haveli, Daman & Diu other than programme States which were identified during 12th five-year plan hence programme was expanded to other 3 States and 1 UTS. Currently the programme is being implemented in 10 States/UTs (7 States and 3 UTs) in total 114 district. The programme is now approved under SFC for FY 2021-22 under the Scheme F of NCDC Umbrella Scheme.

Objective:  Reduction of morbidity and mortality due to Leptospirosis.

Strategies:

1. Development of trained manpower for Leptospirosis diagnosis, case management, prevention &control, and inter-sectoral coordination
2. Strengthening surveillance of Leptospirosis in human.
3. Strengthen diagnostic capacity of laboratory in Endemic States.
4. Create awareness regarding timely detection and appropriate treatment of patients.
5. Advocacy for strengthening of patient management facilities in States for improving the preparedness at all levels for managing.
6. Strengthening inter-sectoral coordination at State and district level for outbreak detection, prevention, and control of Leptospirosis

Implementation of the Programme:

11 endemic State and District level activities of the program are being implemented through the National Health mission since FY 2019-20.

Activities Include-

• Procurement of drugs, diagnostic kits, supplies for management of Leptospirosis cases.
• Development of trained manpower for Leptospirosis diagnosis, case management, prevention and control and inter-sectoral coordination
• IEC/BCC- Create awareness regarding timely detection and appropriate treatment of patients.

• National Programme Name: National One Health Programme for Prevention and Control of Zoonoses (NOHP-PCZ)

*Programme details:

The program was approved by Standing Finance Committee during 12^th FYP as Central sector Scheme. In FY 2017-18 programme was under umbrella scheme of NCDC and since then the program is continuing as Scheme B of Umbrella scheme of NCDC. A total of Rs 8.65 Crores was allocated during FY 2017-18 to 2019-20. In the FY 2020-21 the total Rs. 3.41 Crores are allocated for the program. The programme has now been approved under SFC for FY 2021-26 under Scheme D of umbrella scheme of NCDC.

Vision: Institutionalize Structural mechanism for one health in the country at each level National, State District, Block and village level

Mission: To bring all stakeholders from Policy makers till Front Line workers on One Health Platform with Shared Vision and Common Goals

Goal: Protecting communities and minimizing socioeconomic losses due to emerging and re-emerging zoonotic threats

Objective: To operationalize “One Health” Mechanisms for prevention and control of Zoonoses through strengthening Inter-sectoral Coordination among all stakeholders at the National, State, and District and below district level.

Strategies: Utilizing existing surveillance systems in all sectors, roping in Vet/Wildlife institutes as regional Coordinators, Joint Capacity Building programs, Explore networking and mapping laboratory resources & IEC

Components of the programme

1. Institutionalize One Health at National, State and district level.
2. Integrated Manpower development through Capacity Building by providing GIA to Regional Coordinators & partner institutions.
3. Integrated Surveillance
   1. Sentinel Surveillance Sites on Zoonoses
   2. Integrated Web/digital Portal for zoonoses
4. Integrated Community Outreach Program on One Health GIA to Medical/veterinary institution /organization / Professional/Research Bodies

32 States have formed State Level Zoonoses Committee and District level Zoonoses Committee in every district to review the burden of priority Zoonotic disease prevalent in the state and advice the State authorities on specific measures to be taken for prevention and control of zoonotic diseases.

The State and District level Zoonoses comprises members for medical, veterinary, wildlife etc. The State and District level Zoonoses committee is monitoring the progress of efforts undertaken by State and District authorities for prevention and control of emerging and remerging zoonotic diseases.

• NHM programme activity: Snake bite prevention and control ( SBPC )

*Programme details:

To address the need of dedicated National Programme in India to to estimate incidence, mortality, morbidity, and economic burden due to snakebite envenoming burden and to make snakebite envenoming a notifiable disease, the Mission Steering Group (MSG) of NHM approved the inclusion of activities for Prevention and Control of Snake Bites at District and State level under existing components of NHM in activities i.e., training of medical officer/health workers, surveillance and monitoring, meetings for advocacy and intersectoral coordination and IEC activities per existing PIP matrix under NHM. Also, a separate FMR code SI. No. 200 in current year 2023-24 have been issued by NHM for undertaking dedicated activities for snakebite prevention and control. Further strengthening of surveillance for snakebite envenoming is being done through IHIP. Subsequently appointment of state nodal officers for prevention and control of snakebite envenoming has been initiated and so far, nineteen State Nodal officers have been nominated from different states and UTs.

The major initiatives proposed to be undertaken for mitigating the challenges of Snakebite envenoming in India are as follows –

1. Strengthening of Surveillance of Snakebite through Integrated Health Information Platform (IHIP)
2. Capacity building of medical professionals

• Development of Regional venom centers based on prevalent snake species to assure quality Anti Snake Venom across all regions.

1. Addressing the legislative issues of anti-snake venom collection
2. Digital Mapping and Monitoring of facilities with availability of anti-snake venom for ensuring prompt and effective management of snakebite cases.
3. Community outreach activities to spread awareness about snakebite prevention and management.

• Function as WHO Collaborative Center Rabies Epidemiology with following objectives:

• To provide training in the fields of Rabies epidemiology and to collect and analyze rabies surveillance data and to distribute the information to collaborating institutes.

• To contribute to the Global Health Observatory of the WHO (GHO) by providing expert knowledge and available rabies surveillance and related data or information.
